Meet the Team

Suzette Ball, AAPRN, MSN, NP-C

Suzette graduated from Jackson Memorial Hospital School of Nursing in March of 1981 with an Associate Degree in Science and a Diploma of Nursing.  She returned to school in 1999 for her Bachelor Degree in Health Administration from Florida Atlantic University.  After serving as an administrator for several years, Suzette realized that she was more fulfilled in caring for patients than she was with her administrative responsibilities.  As such, in 2006 she returned to Florida Atlantic University for her Masters in Nursing and Family Nurse Practitioner degree.  Suzette obtained her FNP Board Certification in 2008 and re-certifies every 5 years to maintain this status.  In 2021, Suzette applied for, and was granted,  autonomous status to practice within the State of Florida.

Suzette is currently celebrating her 40th year in health care.  The early years were focused on gaining clinical knowledge at Jackson Memorial Hospital in Miami, FL in assignments ranging from med/surg floor nursing, to telemetry and step down patient care.  She then advanced to Trauma/Surgical/Burn Intensive Care as a bedside nurse, charge nurse, and then Nurse Liaison for the Units.  Suzette was recruited to Broward General Trauma Center as a Quality Management Specialist and ultimately became the Trauma Services Program Manager and Nurse Manager for the Trauma Resuscitation Unit.  After graduating with her Family Nurse Practitioner degree, Suzette worked  Internal Medicine in Parkland, FL.  When she relocated to North Florida, Suzette continued her career in Family Practice and added a specialization in management of motor vehicle crash patients. 

Suzette is a native of Fort Lauderdale, Florida who is happily married to Tim, her husband  of 30 years.  In 2016,  they moved to our area to be close to their  new grandson (who calls Suzette ‘Suki’).  Suzette enjoys spending time with family and friends. She loves cooking, hiking, biking and golfing.

Rachel Tax, DNP, APRN

Although originally from Northern Virginia, I began my education at The University of South Florida in 2009 and received my Bachelor of Science in Nursing degree in 2013. After 5 years of bedside nursing, I applied and was accepted to the Doctoral Nurse Practitioner program at The University of Florida. For 3 years I worked hard to earn my Doctorate of Nursing Practice and graduated in 2020. 

I began my nursing career working for several years in the Progressive Care Unit in a Tampa hospital. This is where I learned the basics of nursing, especially how to build an excellent report with my patients. In 2016, I moved to St. Augustine FL, and began work in the Intensive Care Unit at a community hospital. In the ICU I learned to utilize critical thinking skills and began to realize that I could accomplish my goal of becoming a Nurse Practitioner. Now I am proud and honored to take care of the patients in my  practice and am still learning more everyday! 

I live in St. Augustine Beach, with my fiancé as well as our dogs, cat and chickens. I am a third generation Nurse Practitioner, and I am so glad I have support from my mother and my grandmother. I am also an avid reader and I love to ride my bicycle!
