What is an Annual Medicare Wellness Visit?

An AMWV is a medical benefit that is covered by Medicare 100%, leaving no out of pocket costs to you. The purpose of this visit is to create or update a personalized care plan focused on preventing disease and disability. 

What to expect at your Annual Medicare Wellness Visit?

• We will ask you a series of questions about your medical and family history

• Create or update your list of current providers and medications

• Advanced Care Planning

• Take routine vitals, blood pressure, height, weight etc.

• Brief mental health and cognitive questionnaires

• A personalized care plan, including a list of preventive screening such as mammograms and colonoscopies.
